Welcome to Daybreak, Loretto's Adult Medical Day Program. Daybreak serves the medical needs of Central New York adults over the age of 21 in a comfortable and safe day setting up to seven days a week.

Daybreak is designed to help adults over 21 maintain their highest level of independence while continuing to live in their community. Program participants must have a medical need in order to qualify for the services provided by Daybreak, which are covered by Medicaid.

Loretto Health Support Lifeline

A collaborative effort of Loretto and Franciscan Health Management, Lifeline is an easy-to-use, personal response service that gives older adults living at home the peace of mind in knowing that assistance is only a button-push away; 24 hours a day, 365 days a year. When a senior needs help, he or she can just push the Personal Help Button they are wearing either as a pendant or wristband. Within seconds, a caring certified Lifeline monitor responds, assesses the situation and summons appropriate help; whether it's a neighbor, relative or ambulance.

Lifeline is not just for medical emergencies. It can also be used to answer the telephone without rushing to grab a handset and risking a fall. With Lifeline, older adults and those with limiting medical conditions can have a stronger sense of well being. Their families can also rest assured that help will be immediate. McAuliffe Health Center

McAuliffe Health Center serves individuals in the community as well as those enrolled in Loretto programs. It offers dental care to people of all ages, including children, along with podiatry (the medical specialty of foot care) and some vision care for older adults. McAuliffe Health Center also welcomes patients with Medicaid coverage.

Located in the Cunningham building on the Loretto Brighton Campus at 700 East Brighton Avenue, McAuliffe Health Center is on the bus line with free parking next to the building. Although there is often a waiting list for dental care, in particular, every effort is made to provide timely appointments and comfortable care. PACE CNY is based on the national PACE model which stands for Program of All-Inclusive Care for the Elderly. It offers a full range of services to adults age 55 and older who have been identified as needing a nursing home level of care, thereby enabling them to remain in their homes with as much independence as possible. PACE CNY embraces a philosophy of care that maximizes the autonomy, independence and dignity of each participant. It has two centers located in North Syracuse and on the Loretto Brighton Campus.

With the impending retirement of the Baby Boom generation, experts believe the PACE model represents the future of eldercare in the United States. Services are provided in the PACE CNY day center, participants homes and a hospital or long-term care facility, if needed. Because PACE offers services through a capitated program rather than fee for service, the financial risk of a higher level of service is assumed by the provider, not the participant.

Individuals can receive any number or combination of services based on the individual care plan developed jointly by the treatment team and participant. Services provided include:

The fees for PACE CNY are completely covered for individuals eligible for Medicaid-only or Medicare and Medicaid benefits. Those not eligible for Medicaid and Medicare can pay a monthly fee through private payment or private insurance (be sure to obtain prior approval).
