Team Managed Care

Team Managed CareKey to the vigorous management of care that is central in the PACE Model is the interdisciplinary team that manages, integrates and provides care. Together team members assess need, plan treatment, provide most care, oversee contract services, monitor care and the participant's changing situation and make adjustments to the care plan as needed on a continuous basis. Care can be provided in a variety of settings— home, day center, nursing home or hospital. At least semi-annually the team reassesses each participant. The team also meets at the beginning of each day to review changes in participants during the previous 24 hours and to make any adjustments to the care plan that may be indicated. Thus, the team's care planning is continuous.

NOTE: The PACE CNY team strives to preserve and support the family unit.

In Home Care 

Home Care services include nursing and home health aide care and services. There is a doctor and nurse on call during the evenings and weekends, so participants have access to care 24 hours a day. Home Care visits are made as medically necessary. Some of the duties of a Home Health Aide are to assist with bathing, dressing, and medication reminders. Other Home Care services include Meals on Wheels and Lifeline.

PACE CNY provides services at many locations throughout the community including private homes, apartments and senior housing facilities.

Rehabilitation Rehabilitation

The rehab team includes physical, occupational and speech therapists. The rehab team works with the participants to restore or maintain safe ambulation, transferring, and toileting, and addresses activities of daily living (bathing, dressing, etc.). The PACE CNY team evaluates and provides medically indicated supplies, appliances, and equipment. The team is involved in incontinence prevention, fall prevention, and wound care. Some programs may include aqua therapy, massage therapy or participation in low vision groups. Rehab services may be provided in the Day Center, the home or a skilled nursing facility.

Medical Care & Services 

Medical Care and ServicesThe PACE CNY doctor, nurse practitioner, and nurses are available 8:00 a.m. to 5:00 p.m. Monday through Friday in the Clinic. There are treatment rooms, a medication room, and a room dedicated to massage therapy in the Clinic. The participants are able to have blood drawn, x-rays taken, and cardiograms completed at the Clinic. The medical team reviews all medications upon enrollment. Our team approach focuses on prevention and catching problems early, and we are committed to caring for participants in a non-institutional setting. PACE has a full complement of specialty medical physicians in the PACE CNY network; such as cardiology, pulmonology, rheumotology, etc.

Medication Management 

PACE CNY provides all medications, including over-the-counter drugs, which have been prescribed by the PACE CNY primary care or specialty physician providers. PACE CNY is considered a Medicare Part D plan, therefore, once you join PACE CNY you must disenroll from your existing Part D plan. PACE CNY must coordinate, order, and pay for all of your prescriptions as prescribed. Medications are pre-poured weekly, or filled monthly depending on the participants’ needs.

Day Care / Recreational Therapy 

Day CareThe Day Center Manager coordinates the activities in the Day Center. The Recreational Therapist develops a monthly calendar offering a variety of activities such as bingo, cards, dance, music, exercise class, cooking, games, sewing, knitting, computer classes, etc. They also plan outside trips to shopping centers, restaurants, and special events like the New York State Fair, Lights on the Lake, and the Lilac Festival in Rochester.

The personal care area at the Day Center has a walk-in shower as well as a whirlpool bathtub, for those who have special personal care needs. Home Health Aides are available to assist with activities in the Day Center.

A hairdresser is also available (for an additional fee).

Recreational TherapyThe Registered Dietician coordinates nutrition care by recommending appropriate therapeutic diets and nutritional supplements. The dieticians monitor weight, consumption of meals, and adjust menus in the day center to meet participants' individual needs. Nutrition education is provided on an individual basis.

The Social Workers help coordinate the psycho-social needs with the participants, their families, and the PACE team. They are instrumental in helping address participant or family concerns as they arise. Additionally, as indicated, social work and dietician services may extend beyond the Day Center into the participant's home or skilled nursing facility.

Transportation Services 

Transportation ServicesPACE CNY has its own transportation for PACE participants to take them to and from the Day Center, Clinic, outside doctor appointments, outside activities, and aqua therapy. They also transport participants to St. Joseph's Hospital and the contracted skilled nursing facilities.
