Family & Caregiver Support

The Heritage’s resident service coordinator meets with each prospective resident and their family prior to admission so that families can communicate their loved-one’s history and verbalize their concerns.

Our resident service coordinator is available to families for support, and for assistance with benefits and services. A family meeting is arranged within 30 days after admission to discuss the resident’s care plan and any family or staff concerns. Our social worker continues to be available for questions or concerns.
