Services

Daybreak’s interdisciplinary care plan team provides integration and coordination between our participants and their personal physicians in order to insure that all aspects of an individual’s needs are addressed.

Skilled Nursing Intervention, provided by a licensed practical or registered nurse (LPN or RN), includes medication administration, injections, Individual health assessments, chronic disease management, health education and skill training, health monitoring, wound care, respiratory services, tracheotomy care and monthly vital signs.

Qualifications

To enroll in Daybreak, participants must be age 21 and older with a medical condition warranting a minimum of weekly health care or rehabilitative therapies ordered by a physician.  There is no need to change your current doctor. We will work with him or her.

There is a minimum enrollment requirement of five hours per day, one day per week at Daybreak.  However, participants can attend anywhere from one to seven days a week.

Special Care Programs

Acute & Chronic Respiratory Illness

Our skilled nursing staff assists with managing respiratory illnesses whether chronic (COPD) or acute (bronchitis, upper respiratory infections and pneumonia). Participants who live with Chronic Obstructive Pulmonary Disease (COPD) struggle daily to breathe and, although there is no cure, it is a manageable and treatable disease.

Diabetes

Participants who live with diabetes are of special concern to Daybreak. To provide them with the best care possible, we have developed the Daybreak Diabetes Management Program, geared to meet each individual’s needs. Our skilled staff focuses on incorporating diabetes management into our patients’ daily routines.

Kidney Disease

Kidney disease is often difficult to manage, especially if it is accompanied by high blood pressure and diabetes. Loretto's Daybreak program helps our participants remain independent while assisting them with their health care needs. Our professional staff assists participants in maintaining health and improving quality of life.

Wound Care

Daybreak’s health care professionals understand the intricacies of dealing with open wounds and pressure sores.
Our qualified staff helps participants to manage their wounds by promoting healing and preventing infection.

Pharmacy/Medication Review

A licensed pharmacist provides pharmacy/medication reviews. Each participant’s medication list is reviewed bi-annually for appropriateness.

Nutritional Services

A registered dietician assesses, educates and assists participants with nutritional needs by coordinating and providing individualized diets that meet their medical needs as directed by their physician.

Case Management

Case management at Daybreak is overseen by a MSW (Master of Social Work). Assessment and coordination of community services include housing, home care, assistance with entitlement programs and psychiatric counseling.

The case management services at Daybreak ensure that each participant has access to necessary services that will assist them in living in the community safely and appropriately.

 Therapeutic Recreation

Therapeutic recreation at Daybreak is overseen by a certified therapeutic recreation specialist.  It's a process that utilizes treatment, education, and recreation services to help people with illness, disabilities, and other conditions to develop and use their leisure in ways that enhance their health, functional abilities, independence and quality of life.

Ongoing daily activities geared toward wellness and independence include:

  • Cognitive programs

  • Weekly outings
  • Special events
  • Creative arts

Physical, Occupational and Speech Therapies

Physical Therapy

Physical therapists provide medically prescribed physical therapy treatment focused on improving or restoring functions that have been impaired, reduced, or permanently lost by illness or injury.

The services provided by the physical therapists at Daybreak prevent the onset and progression of functional limitations, and promote quality of life and functional mobility through therapeutic exercise for strengthening, coordination, balance, endurance, lower extremity range of motion and lower extremity orthotic and prosthetic training.

Occupational Therapy

The services provided by occupational therapy at Daybreak promote quality of life and functional mobility through:

  • therapeutic exercise
  • upper extremity strengthening
  • upper extremity range of motion
  • fabrication of upper extremity splints
  • upper extremity orthotics and prosthesis training
  • safety education
  • application of physical agents (paraffin and fluidotherapy) to alleviate pain
  • improved activity of daily living (eating, grooming, dressing, toileting and bathing, etc.) by adapting the environment or use of adaptive equipment, if necessary
  • Improved instrumental activities of daily living (cooking, cleaning, laundry, etc.)

Speech Therapy

Speech therapy at Daybreak features a speech-language pathologist who is a health care professional educated and trained to evaluate and treat people with speech, language, and swallowing problems.

Personal Care Services

Certified nurses aides provide personal care services at Daybreak including whirlpool baths and showers (handicapped accessible), catheter and colostomy care and toileting/transfer assistance.

Transportation Coordination

Daybreak coordinates transportation for participants to and from the program, weekly outings and provides transportation assistance for medical appointments.
