Food and Nutrition
The Commons on County House Road's Dietary Department serves residents attractive, nutritional meals based on the Recommended Daily Allowance. The Diet Tech and Dietary Manager work with the Resident Council to plan seasonal menus. Meals are normally served buffet style in the dining room. Tray service is also available for those who wish to remain in their rooms. Special occasions, such as holidays, are occasions that call for special treats.
Modified diets are provided when ordered by a physician. We have on-floor stations for between meal nourishment that is tasty as well as nutritional. Our Dietary Tech and Manager are available to discuss meals with residents and family members and to provide encouragement for an adequate food intake. The Registered Dietician is on-site on a weekly basis and available for those on special diets.
The Commons on County House Road contracts with AdvancedMeal to provide our food and clinical nutritional services.
The Commons on County House Road Rehab Department's goal is to provide residents with the support to maximize their abilities during their stay at the facility. In addition, we want to rehabilitate and prepare short-term patients for their transition to their homes or to the next level of care.
We provide Physical, Occupational, and Speech Therapies 5 days a week. Our therapy room is equipped to provide treatments for most conditions. The goal of therapy is for patients to achieve the highest level of independence possible.
Physical Therapy sessions last 30 to 40 minutes. Therapists put an emphasis on walking and developing the strength needed to get out of bed or in and out of a chair. To assist with these capabilities, therapists use parallel bars, exer-cycle, and weights to achieve goals established by a physician.
Occupational Therapists work on upper body strength and everyday skills such as bathing, dressing and eating. Sessions last 30 to 60 minutes.
Speech Therapy is available on an as needed basis. Therapists provide services for those who may have a speech deficit or who may be in need of evaluation for a swallowing problem.
All admissions to The Commons on County House Road are handled by our Admissions Department. Our staff is here to help you with any questions you may have about the admissions process. A required assessment, Patient Review Instrument or PRI, must be completed prior to admission. Contact Admissions Director Judy Greco 253-0351 ext 244 for more information.
